First, you need a wreath form. I like the 18 inch straw forms, the straw ones are cheaper than the styrafoam and I like the lumpy look of the straw ones. Mine was $4.99 at Hobby Lobby.
Update: keep the plastic wrap on the wreath (that is if you get a straw one, I think it helps keep everything in tact)

Then, get your hot glue gun out and ready. Make a good sized circle of glue on the back of the wreath then put the start of your yarn in the hot glue and hold until the glue cools. (Does that even make sense!)
Once the glue is dry, start wrapping!!!!! (keep the yarn taught as you wrap so that it stays where you want it)
Go ahead and get into some comfy clothes.
And wrapping,
And, holy shit, were're still wrapping. Brian photo-bombed my picture!
Seriously, I think I watched a whole season of The X-Files while wrapping this one! I love you David Duchovny! Sometimes the straw wants to poke through your yarn, I just push the straw in with my fingernail.
Finish the wrapping, look over the wreath to make sure you didn't leave any gaps. If you did, I just wrap it around again strategically until I cover all the gaps. Then do the same as you did earlier, make a dot of hot glue and pull the yarn through it and hold until the glue dries. Then clip off that excess yarn.
Now the best part hot gluing the "decorations" on it. YAY! Pick out some stuff from a craft store. I get a lot of my stuff from Hobby Lobby. I lay the items on the wreath before I glue so that I can see roughly what it will look like and if I like it. There is a dab of hot glue under my thumb and right above it. Once that dries I add a little more on top to make sure it is securely attached to the wreath. I don't worry about the hot glue showing because I use big flowers and their petals hide the glue.
Add another stem. Sorry guys, I didn't go picture by picture here. I like to use two different colored stems so a gold and red on both sides. The flower will go in the middle. Glue each stem down the same as before. OMG, do not look at the mess that is my house in that bottom picture! Now that I said that you will, just don't judge.
Now, the flower. Which can sometimes be a pain, this one was, which is why I don't have a picture because I had to hold it in place until the glue dried. Which seemed like forever!!!!!!! So, however you can get that thing on. By the way, a lot of my flowers had stems and I look before I buy them to make sure I can pull the flower part off the stem!
Tada! You're done and you have a beautiful wreath.
